BACKGROUND INFORMATION

Your participation in the survey is voluntary, and you may stop participating at any time.

CONFIDENTIALITY

All of your responses are equally important and will be kept confidential. The services you receive will not be impacted by your answers or by refusal to participate.  The data from this research will only be reported as a collective, combined total, and no one will have access to your individual information. The information you provide will only be used to produce dependable and vital statistics for the Tribe.

Your name will not be collected. All of your responses will be held and kept confidential. This information cannot and will not be used to limit or change your eligibility for services or assistance.
